Are you sure you got global scoped Ipv6 addresses on eth5 and switch0? Check out the config I posted yesterday. Works for my friend with that dhcp-pd and firewall config. As I said, I'm not sure if limiting the source port for dhcpv6 works. My friend had problems as well, then we simplified the icmp rules (allow all, as my config allows minimal icmp traffic for ipv6 to work only) and removed the source port for dhcpv6 and it worked. Not sure what fixed it to be honest.
